Meningitis Vaccine in India: Who Should Get It According to IAP Guidelines?

Meningitis is a fast-progressing, life-threatening infection that can cause brain damage, hearing loss, and even death. Early protection is crucial, especially for infants and young children. Menactra, given from 9 months of age, helps protect against four major strains of meningococcal bacteria, making timely vaccination an important step in preventing severe disease.

Meningococcal meningitis is one of the fastest-progressing bacterial infections, capable of causing severe illness within hours. It can lead to meningitis (infection of the lining of the brain and spinal cord) or meningococcal septicemia (bloodstream infection), both of which can be life-threatening even with prompt medical care.

Fortunately, vaccination offers highly effective protection against the major disease-causing strains of Neisseria meningitidis.

Does Every Child in India Need a Meningitis Vaccine?

According to the Indian Academy of Pediatrics (IAP) Advisory Committee on Vaccines and Immunization Practices (ACVIP), meningococcal vaccination is not recommended as a routine vaccine for all children in India. This is because India has a relatively low overall incidence of meningococcal disease, although outbreaks can occur in certain regions and settings.

However, the vaccine is strongly recommended for children and adults who are at increased risk.

Who Should Receive the Meningococcal Vaccine?

The IAP recommends meningococcal vaccination for:

  • Children with anatomical or functional asplenia (including sickle cell disease)
  • Children with persistent complement deficiencies
  • Individuals receiving complement inhibitor therapy (such as eculizumab or ravulizumab)
  • Children and adolescents with HIV infection
  • Individuals exposed during a meningococcal disease outbreak
  • International travellers, especially:
    • Hajj and Umrah pilgrims
    • Travellers to the African meningitis belt
    • Students going abroad to countries where vaccination is mandatory or strongly recommended
  • Certain laboratory personnel who routinely handle Neisseria meningitidis

Which Vaccines Are Available in India?

Quadrivalent meningococcal conjugate vaccines (MenACWY) protect against four major disease-causing serogroups: A, C, W, and Y.

The commonly available conjugate vaccines include:

  • Menactra®
  • Menveo®

The IAP prefers conjugate vaccines over older polysaccharide vaccines because they provide stronger, longer-lasting immune protection and help reduce bacterial carriage, contributing to community protection.

What Is the Recommended Schedule?

The schedule depends on the child's age and risk category:

  • 9–23 months: Menactra is given as 2 doses, at least 3 months apart.
  • Children aged 2 years and above: A single dose of MenACWY conjugate vaccine is recommended.
  • Children who continue to remain at high risk may require booster doses every 5 years. For children vaccinated before 7 years of age, the first booster is recommended after 3 years, followed by boosters every 5 years while the risk persists.

Important Vaccination Note

For children with functional or anatomical asplenia receiving Menactra, the IAP recommends administering the pneumococcal conjugate vaccine (PCV) first and giving Menactra at least 4 weeks later. This timing recommendation does not apply to Menveo.

Why Early Protection Matters

Although meningococcal disease is uncommon, it can progress rapidly and may cause:

  • Meningitis
  • Bloodstream infection (septicemia)
  • Hearing loss
  • Brain injury
  • Limb loss
  • Death within hours in severe cases

Vaccination provides the best available protection for individuals at increased risk.
